As the primary coordinator of the Elder at Home care team, the RN-Care Manager provides across-the-continuum, customized care coordination services and management of transitions of care for the most medically fragile PHP Medicare Advantage, at-risk patients and their families/caregivers, incorporating patient-centered interventions and patient engagement strategies to promote self-management in the home/community setting.

Required Qualifications: 

Oregon Registered Nurse License upon hire. 2 years Clinical medical-surgical, acute care or long-term care experience that also includes:      1 year in community-based/ambulatory care.      1 year in care of the frail elderly.

Preferred Qualifications:

Bachelor's Degree in Nursing. Home health, hospice, palliative care, managed care, or skilled nursing facility background. 1 year Care management of diverse patient populations.
