**Job Description** Founded in 1921, Straub Medical Center includes a 159-bed hospital in Honolulu, a network of neighborhood clinics and a visiting specialist program that reaches throughout the state of Hawai'i. Under the direct supervision of the department Manager or the designee, provides care management and care coordination for patients enrolled in the Care Management Program in the Primary Care setting in partnership with the primary care practice health team under Hawai‘i Health Partners. Will serve in an expanded health care role to collaborate with specialists, members of the health care team and patients/families/caregivers to ensure the delivery of quality, efficient and cost effective health care services. Assesses care plans, implements, coordinates, monitors, and evaluates all options and services with the goal of optimizing the patient’s health status. The Administration team works to ensure that all these services are delivered and managed in an effective manner with friendly customer service. As the Care Management Services Registered Nurse, you will provide care management and coordination for patients enrolled in the Care Management Program in the Primary Care setting, in partnership with the primary care practice health. You will serve in an expanded health care role to collaborate with specialists, members of the health care team and patients, families and caregivers, to ensure the delivery of quality, efficient and cost effective health care services. Current driver’s license and valid Hawai‘i auto insurance with short notice access to an automobile to conduct site visits, as required. Two (2) years of clinic experience in a Primary Care setting. Experience working within a collaborative interdisciplinary team. **Preferred Qualifications:** Certification in Case Management. Experience in care coordination for patients.
