The Non-Clinical Case Manager coordinates referrals, serviceplanning, and client documentation for designated caseload(s).

Where: Waukesha County What: Care Manager – Elderly and Disabled Populations Start date: ASAP Length of Contract: 6 month temp to hire Pay: $27/hr Shift: 1st Hours: Monday – Friday; 40 hours Requirements: Bachelor’s Degree, Previous experience with Elderly and/or Disabled Adults.

Minimum Requirements:

Professional License in Behavioral Sciences, Human Services,or Social Services preferred Bachelor’s Degree in Behavioral Sciences, Human Services, orSocial Services fields preferred Experience in Case Management, Behavioral Sciences, HumanServices or Social Services preferred Current CPR if applicable TB questionnaire, PPD or chest x-ray if applicable  Current Health certificate (per contract or stateregulation)  Must meet all federal, state and local requirements  Must be at least 18 years of age
