Summary:
Serves as a resource for planning, coordinating and facilitating coworker education. Responsible for the planning, coordinating, and facilitating education initiatives in support of Care Management mission, and vision. Serves as a role model, resource and advocate for performance improvement in collaboration with co-workers. Shares knowledge and skills with staff and provides constructive feedback. Assures accurate and compliant documentation is occurring through routine auditing. Investigates potential quality issues and reports findings. Coordinates and educates on changes needed. Identifies and recommends performance improvement opportunities reporting results with the management team to improve the overall care of patients.
Education:
Must have a BSN from accredited school of nursing. Acceptable accrediting agencies are Commission on Collegiate Education in Nursing (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA). Master’s degree in healthcare related field or MSN preferred.
Licensure/Certification:
Must have a current Indiana state license as an RN.
Experience:
3-5 years of nursing experience. Leadership experience preferred. 3-5 years care management experience preferably in Home Health, Ambulatory Care, Community Public Health, Case Management, and Coordinating Care across multiple settings and with multiple providers.

Exceptional communication skills, both written and oral, ability to positively influence others; Requires ability to communicate with physicians, other health care providers, families and patients. Strong work ethic built on a foundation of proactivity and team work. Ability to navigate ambiguity with the aid of structured problem solving techniques.
