As a Care Navigation Assistant, you will:
• Process skilled nursing facility (SNF) referrals quickly and effectively through and to admission or non-admission using a variety of systems and resources including (but not limited to Cerner and Care-Port) managing phone calls, managing the internal fax system, and monitoring/processing electronic systems for new referrals and moving referrals, obtaining appropriate documentation for the completion of the referral and documentation of the status in Payor Portal, Cerner and Care-Port
• Support the AHC Acute Care Hospital Care Navigation teams by processing acute and Post-Acute referrals, follow up, and other duties supporting the transition of persons to acute and Post-Acute services (i.e. initiating and following up on authorizations
• Monitor Care-Port for authorization requests and updated tasks
• Collaborate and communicate with Clinical/non-Clinical liaisons pertaining to referrals using Care Port, fax and/or telephone
• Provide the Important Message from Medicare-to-Medicare Inpatients in accordance with CMS guidelines
• Provide the Observation Notification Communication to all patients admitted in Observation status
• Complete required documentation in a timely and accurate manner
• Support RN/SW Care Navigator with patient who require transportation support. To include BLS/ALS ambulance request, wheelchair, and/or ride share. • The completion of transportation authorizations, payor forms, vendor specific forms, and MOLST forms with required documentation
• Other duties as assigned (may include following up on face-to-face documents and assisting with Medicare appeals)
Qualifications include:
• High school diploma or equivalent required. AA Degree or Higher in Business or HealthCare preferred.
• 2-3 years’ experience working in a medical setting.
• 100% of job in the Hospital Setting.
• Excellent customer service and interpersonal skills required.
• Ability to problem solve, multitask in a fast-paced environment with multiple team members.
• Strong understanding of health care terminology.
• Experience with data entry.
• Self-directed, ability to work independently and confront challenges, exercises good judgment.
• Excellent verbal and written communication skills.
• Proficient in standard computer business applications (i.e., Microsoft Office Suite).
• Empathic toward individual’s complex health needs and their families.
• Strong organizational skills: able to develop workflows to create more efficient processes.
• Excellent time management skills.
