Care Navigator
Compensation: $16.80 - $21.00 per hour (based on years of experience)
Summary:

The Care Navigator plays a pivotal role within the Care Coordination Division of the Quality Department. Reporting directly to the Care Coordination Manager, the Care Navigator is an integral member of collaborative pods alongside Care Coordinators. This position is focused on enhancing patient care journeys by providing support, guidance, and coordination to ensure seamless transitions through various healthcare services.

Principal Responsibilities:

1. Will participate and maintain a culture within The Corvallis Clinic that is consistent with the content outlined in the Service and Behavioral Standards document. To this end, employee will be expected to read, have familiarity, and embrace the principles contained within.

2. Assists Care Coordinators to schedule appointments, diagnostic tests, and consultations as required. This can include assistance in arranging transportation or logistical support for patients who require assistance.

3. Performs tuck in, check in and follow up calls to high-risk patients, providing support to patients with education on self-care techniques, disease management strategies, and methods for actively participating in their treatment plans as directed by Care Coordinator(s).

4. Updates resource lists for patients; identifying relevant community resources, support programs, and services that align with patients' needs and preferences, and assists patients in accessing these resources to bolster their overall well-being and facilitate recovery.

5. Engages with patients to understand their medical history, treatment plans, and care goals and works in tandem with assigned Care Coordinator(s) to develop relevant plans and approaches to care.

6. Ensures accurate and timely documentation of patient interactions, interventions, and outcomes.

7. Provides emotional support to patients and their support person(s), addressing their concerns and addressing any uncertainties they may have.

8. Facilitates communication between patients and healthcare providers, ensuring that all parties are well-informed about care plans and progress.

9. Serves as a vocal advocate for patients' rights, preferences, and needs within the healthcare system, and escalate concerns or challenges faced by patients and their support persons to appropriate parties.

10. Participates in team meetings and contributes to the collective goal of optimizing patient care.

11. Collaborates within the pod team to identify potential challenges or barriers to effective patient care and contributes to innovative solutions and suggestions to improve processes and patient experiences.

12. Occasionally works at other Corvallis Clinic Sites as assigned.


Education/Licensure/Experience:

1. High school diploma or equivalent required.

2. One of the following is required:

                -  National certification as Medical Assistant, LPN, or CNA II.

                  - Advanced coursework in a healthcare-related field.

                  - Minimum two (2) years of experience working in community or health care setting.

3. Experience in the following preferred:

                -  Outpatient, ambulatory care and/or community care

                 - Care coordination

                 - Electronic medical record (EMR) system


Knowledge and Skills:

1. Knowledge of healthcare systems, medical terminology, and patient care processes.

2. Demonstrated initiative, ability to work with others in an engaging manner, and good professional judgment.

3. Proficiency in using relevant software and tools for documentation and communication.

4. Empathetic and patient-centered approach to care navigation and demonstrated commitment to impacting the care of high-risk patients.

5. Excellent organizational skills and attention to detail for accurate documentation and coordination.

6. Problem-solving ability and adaptability in a dynamic healthcare environment.

7. Strong communication skills to effectively interact with patients, colleagues, and healthcare providers.
