Position Summary

The Care Navigator & Data Collection Coordinator for the Medication Assisted Treatment (MAT) program is an integral member of a multidisciplinary community network, providing clerical/administrative coordination, data collection to meet Grant requirements and is the point of contact for patients with opioid use disorders in receiving medication assisted treatment, chemical dependency, behavioral health, and support services. This position involves interaction with the patient and professionals and requires professionalism, good judgment, discretion, good public relations, customer service, data collection/collation/reporting & data management skills, clerical and organization skills. This position requires extensive interaction with both internal and external contacts.

Requirements

Vocational business training or college course work OR equivalent experience 2 years in health care setting Addiction and/or related experience in medical/social issues preferred

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $20.47 - $29.45 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
