The Care Navigator for Population Health Services Organization (PHSO) responsibilities include:

The Care Navigator PHSO is an embedded member of a collaborative practice model that includes primary care clinicians and office staff, patients, caregivers, the PHSO and the community. They will develop and maintain relationships with high-risk members acting as the contact point in navigating their health and social needs in collaboration and partnership with the physicians and PHSO clinical programs. This will include a strong Health Coaching component, where transparent and motivational discussions incentivize patients to engage in the programs and resources offered to them. The Care Navigator will also assist members with community resource navigation by researching, finding, and linking patients with resources, services, and support mechanisms for their care plan and self-management needs.

Required Education

LPN OR bachelor's degree in social work (or related field)

Required Credentials

LPN (Licensed Practical Nurse) licensure in lieu of bachelor's degree in social work (or related field)

Required Experience

3 years' experience in Care Coordination, Health Coaching, and/or Physician Office AND 1 year experience in health care
