Responsibilities: 

Complete Admission, Re-Admission, Annual, and Significant Change MDS Care Area Assessments (CAAs), ensuring compliance with state and federal guidelines for resident-centered care plans.Develop and implement comprehensive care plans, including measurable objectives, time frames, and services needed to support residents’ physical, mental, and psychosocial well-being.Prepare and communicate Baseline Care Plans within 48 hours of admission, detailing initial goals, care instructions, and medications.Lead care plan meetings, coordinating with the interdisciplinary team (IDT) to ensure individualized care and involving residents, families, and representatives in the process.Maintain up-to-date care plans reflecting current clinical data, ADLs, and advance directives; collaborate with Nurse Managers and attend daily and weekly care meetings.Conduct quality assurance audits, daily clinical rounds, and actively engage with residents and families to uphold high standards of customer service and quality care.

 

Job Type: Full Time

 

Salary: $48.00 per hour

Qualifications

Education:

High School Diploma or equivalent. College degree preferred.

Experience Requirements

Minimum 1 year of MDS experience preferred.

License Requirements
