Care Transition Specialist (CTS)

 Work hours: Full-time, 40 hours/week; Monday to Friday Between 8:00AM-5:00PM, 8-hour days

Here’s What You Need

·        High School Diploma or Equivalent Required

·        3 Years relevant experience Required, (preferably in a healthcare or service environment)

·        Knowledge of medical terminology preferred

Problem solving skills Latitude for decision making and setting of priorities 

·        Interpersonal skills and Communication skills

·        Ability to work with minimal supervision or without supervision when necessary

·        Proficient computer skills with experience in: MS Outlook, MS Office (Word, Excel), internet navigation, clinically-based software, and financially-based software 

 

What You Will Do

•        Responsible for coordinating the clerical and administrative processes for entity care transitions management

•        Responsible for managing departmental mailbox, phone lines and/or fax

•        Supports transition planning activities as needed under the direction of care transition managers: Calls vendors, physician offices, clinics, etc.; Faxes (electronic and traditional) documents as needed

•        Schedule follow-up appointments, call Insurance companies for referral approvals, and sets up transportation for patients

•        Assist/support the Case Managers – will be assigned to 3 to 4 floors of patients – will go into the patient’s rooms for signatures on documents, as well as deliver the Durable Medical Equipment/DME (ie. Wheelchairs, walkers, wound vacs) to the patients prior to discharge

•        Makes copies or gathers other data as needed

•        Collects and enters data as needed

•        Completes other tasks and projects as assigned
