The Case Manager/Healthcare Training will manage a caseload of program participants that enrolled in Bon Secours Community Works’ Healthcare training program. The position will ensure that all clients have a positive training outcome by ensuring the effective implementation of recruitment strategies, enrollment, education, case management, and reporting. The Case Manager will work with clients in a one on one and a group setting with the goal of reducing barriers and placing them into careers.

Manage a caseload of program participants that are enrolled in Bon Secours Community Works’ Healthcare Training Program.

Executes performance goals and a work plan that is outlined by the Program Manager.

Ensures that all participants receive effective case management services, financial coaching, job preparedness education, and employment resources.

Works with the Program Manager to refine all aspects of program design and supports with documenting all amendments it in a standard operating procedure.

Adheres to the guidelines, policies, and procedures detailed in the program standard operating procedures.  

Ensures that all data reporting is completed by assigned deadlines for funders, Program Manager, and Director.

Inputs all records, metrics, and outcomes in Apricot and/or additional databases.

Work collaboratively with the Program Manager to improve and expand the Healthcare Training Program.

Works collaboratively with the Program Manger to monitor progress towards meeting grant performance metrics.

Maintain key partnerships that increase client outcomes.

Participate in outreach, recruitment, and engagement activities in the community. 

Facilitate client development groups using evidence-based manuals and curriculums. 

Deliver presentations and communicate programmatic strategies when assigned.

Coordinate events to engage current and completed participants

Education Qualifications -

Required Minimum Education: Bachelor’s Degree Specialty/Major Psychology, Social Work, Business, Human Services, Human Resources, Education or equivalent from an accredited college or university or equivalent experience is required

Preferred Education Masters Degree Specialty/Major Business, finance, social work, public health, human services, education

Minimum Qualifications

Minimum Years and Type of Experience 1 year of professional experience in delivering workforce development training programs. 

Other Knowledge, Skills and Abilities Required Organizational skills, record keeping skills, and time management skills

Other Knowledge, Skills and Abilities Preferred Proficiency in oral presentations, writing skills, and Microsoft Office Suite.

Other Knowledge, Skills and Abilities Preferred Effective verbal, oral presentation, and written communication skills.

Other Knowledge, Skills and Abilities Preferred Strong interpersonal skills necessary to develop and maintain relationships with diverse stakeholders. 

Other Knowledge, Skills and Abilities Preferred Detail oriented with excellent critical thinking skills
