Overview

The Companion Caregiver plays a vital role in enhancing the quality of life for individuals in need of companionship and support. This position focuses on providing engaging activities, emotional support, and practical assistance to promote social interaction and overall well-being. The ideal candidate will be compassionate, patient, and enthusiastic about building meaningful relationships with clients.

 

Key Responsibilities:

Companionship: Provide emotional support and companionship through fostering a trusting and caring relationship.

Activity Planning: Organize and participate in recreational activities such as card games, pottery classes, arts and crafts, and light workouts tailored to clients’ interests and abilities.

Social Engagement: Encourage social interaction by facilitating discussions, storytelling, and outings to community events or local attractions.

Daily Living Support: Assist with grocery runs, meal planning, and tasks as needed, ensuring clients' environments are comfortable and safe.

Transportation Assistance: Accompany clients to appointments, social events, or errands, providing safe and reliable transportation.

Health and Wellness Activities: Lead or participate in light exercise routines, ensuring clients remain active and engaged in their physical health.

Communication: Maintain open and effective communication with clients, their families, and healthcare providers to ensure a holistic approach to care.

Shifts: OVERNIGHT 2:30 - 2:30 

 

Responsibilities Performs comprehensive evaluations, assessing the needs of the patients, and sets measurable objectives and goals that are formulated in conjunction with the rehabilitation teamAdjusts treatment to achieve maximum results in independence and/or functionPlans, organizes, and coordinates recreation activities, functional leisure skills, and leisure educationInterviews the patient to assess capabilities/interests/barriers. Monitors progress and recommends changes/modifications to program as necessaryDesigns community reintegration activities to assist the patient with their physical and cognitive needs during physical recreational/leisure activity Assists the patient in understanding their various needs and limitations and how to manage them during functionRegularly re-assesses effectiveness of treatment plan, progress of patient towards goals and the need for modifying goals or treatmentCommunicates patient’s needs and progress to the treatment team, physician, patient, family members, and others as applicableObserves/documents and reports changes in the condition/status of the person receiving services to the Clinical Coordinator, Program Manager or DCM, attending physician and other treatment team membersConsults with medical providers regarding patient evaluation & treatment, progress of the patient & discharge from treatmentMaintains and submits evaluation, treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulationAdheres to standards of the Company's Code of Business Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal lawsMay work with rehabilitation specialists in fulfilling the patient’s treatment plan through instruction in patient-related activities to be carried out by the rehabilitation specialist with supervision provided by the Therapeutic Recreation SpecialistPerforms other duties as assigned Qualifications Bachelor’s degree in Therapeutic Recreation Therapy, or the equivalent Maintains certification as a Certified Therapeutic Recreation Therapist (CTRT) through the National Council for Therapeutic Recreation Certification (NCTRC)Maintains professional license and/or certification if required by the state in which the CTRS practicesMinimum of two years therapeutic therapy experience and therapeutic recreation program designExperience working with patients with neurologic or orthopedic injuries or conditions Demonstrates effective and professional interpersonal, verbal, and written communication skillsAble to work independently and part of a teamDemonstrates strong organization skills and attention to detailHas reliable transportation and can travel to/from multiple assignments/locations on a daily basis About our Line of Business Rehab Without Walls is a revolutionary neuro rehabilitation program that moves individuals outside the walls of institutional settings and into their natural home and community environment through a continuum of care.
