To provide comprehensive clinical case management services that include: assessments; individual, group and family treatment; treatment plan coordination; referent relations; discharge planning; and intervention with client support systems, including social service programs and managed care. Requirements High school diploma, general education degree (GED) or bachelor’s degree with CASAC-T, CASAC certification or LMSW, LMHC. Salary Description $22.39-$35.00/ hour
