Hourly Rate: $15/hour

Deliverables/Principal Results Expected:

Responsible for identifying/assessing client needs and making the appropriate services available. Coordinates all appointments, aftercare plans, schedules and meetings. Performs pre-certifications, continued stay, discharge reviews, PCPCs, lost funding causes as needed. Ensure client chart information is maintained and up to date Obtains the necessary information and releases from client, family, referral sources and other appropriate sources. Support aftercare referral/placement outreach and establishment Participate in implementing client’s treatment: maintaining communications with clients, family members and counselors. Coordinate transportation needs for external appointments, accompany, and transport clients as assigned. Oversee daily schedule for 3C clients. Provide individual and family sessions and educational groups and workshops. Other duties as assigned.
