Coordinates and implements the transition of care (TOC) / Discharge (DC) plan for ambulatory patients. Prioritizes and coordinates the plan across the care of continuum through critical thinking, teamwork, and communication between care providers, patients, families and external vendors to ensure timely discharge. **Job Description** : _These Principal Accountabilities, Requirements and Qualifications are not exhaustive but are merely the most descriptive of the current job.
