The Case Management Assistant responsibilities include:

Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute careResponsible for assisting with distributing regulatory documents to identified patientsEducating patients about the appeal process related to their dischargePerforming multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community serviceRequires frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives

Location: St Anthony's Hospital - St Petersburg, FLStatus: full time, 40 hours per weekSchedule:  Friday, Saturday, Sunday, Monday - 10 hour shifts                       7:00 AM - 5:30 PM    OR    8:00 AM - 6:30 PMWeekend Requirement: EveryOn Call: No

 Education:

Required High School or EquivalentPreferred Associate's

Experience:

Required 1 year in Medical Related Field
