Compensation varies based on factors including, but not limited to, experience, skills, education, performance and salary range at the time of the offer._ **Responsibilities** Lead and direct case management services, workflow and resources. + Formulate and implement operational strategies and initiatives in the case management program. + Direct and evaluate all services provided, including care plan development, service level determination, and complaint management to achieve performance and quality control objectives. + Provide specialized expertise, ensure resource alignment with system-wide initiatives, and updates appropriate parties on case management strategies, initiatives, outcomes and issues. + Successfully partners with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care. **Requirements** Licensure / Certification / Registration: + One or more of the following required: + Registered Nurse credentialed from the Florida Department of Health obtained prior to hire date orjob transfer date. + Social Worker credentialed from the Florida Board of Clinical Social Work, Marriage and FamilyTherapy and Mental Health Counseling obtained prior to hire date or job transfer date. + Case Manager credentialed from the American Case Management Association (ACMA) preferred. + Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
