*POSITION SUMMARY:*
The Case Management (CM) Lead is fully engaged in promoting teamwork, prioritizing workflow, addressing or escalating concerns and using resources efficiently to ensure continuity of quality patient care. The CM Lead serves as a professional role model through onboarding and precepting of staff, supporting and encouraging growth and development, and facilitating the mission, goals and objectives of the department.
*PRIMARY ACCOUNTABILITIES *
1. Promotes collaboration and facilitates problem solving with team members, physicians, and administrative personnel.
2. Provides daily direction for peers, onboarding new associates and serving as a role model through attitude, actions, and behavior to reflect the mission, values and daily focus of Health First.
3. Works routine unit assignments as needed to fill in when staffing needs occur.
4. Communicates in a positive, cooperative manner with charge nurses, administrative supervisors, and administration to promote teamwork and optimum patient care.
5. Assists with implementation of department-based performance improvement activities resulting in improved operations and patient care outcomes.
6. Completes assigned audits and monitors accurately and in a timely manner.
7. Identifies opportunities for quality improvement.
*MINIMUM QUALIFICATIONS*
* Education: Associate’s Degree in Nursing or Master’s Degree in Social Work.
* Licensure: RN must hold current State of Florida RN license or endorsement.
* Certification: Must hold current American Heart Association Basic Life Support Healthcare Provider Completion Card upon hire and maintain.
* Work Experience: At least one (1) year Case Management experience in hospital-based acute care.
* Knowledge/Skills/Abilities:
o Strong critical thinking and problem-solving skills.
o Excellent oral and written communication skills with ability to effectively collaborate with a diverse customer base.
o Ability to multi-task effectively.
o Proficient computing skills including use of MS Office Suite applications and online resources.
*PREFERRED QUALIFICATIONS*
* Education: BSN for RN only.
* Licensure: LCSW for SW only.
* Work Experience: Two (2) years of progressive administrative experience.
* Knowledge/Skills/Abilities.
o Current knowledge of Medicare/Medicaid and Managed Care Guidelines.
o Working knowledge of The Joint Commission and other regulatory/accreditation agency rules and regulations.
* PHYSICAL REQUIREMENTS (Sedentary)*
* Majority of time involves sitting or standing; occasional walking, bending, stooping.
* Long periods of computer time or at workstation.
* Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
* May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
* Communicating with others to exchange information.
* Visual acuity and hand-eye coordination to perform tasks.
* Workspace may vary from open to confined, on site or remote.
* May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
