Details: Position hours are Mondays from 10:00am-6:30pm and Tuesdays-Fridays from 8:00am-4:30pm.
The Case Manager Specialist will be responsible for reactive HIV, Hepatitis C, and syphilis result delivery and linkage to care for all patients who are tested in the 5 Emergency Departments located in the Indianapolis Metro region. Tasks will include making phone calls to patients to discuss lab results and coordinate follow up care, visiting patients while they are in the Emergency Department or admitted to the hospital to coordinate follow up care, scheduling appointments, making referrals to other community-based resources, and accompanying patients to appointments and the pharmacy as needed. Other responsibilities will be participating in community outreach events, working with state health department Disease Intervention Specialists to help located individuals who are out of care and reconnect them.
• High School Diploma/GED required. Associate's Degree preferred.
• 1-3 years of experience required.
• Requires the ability to comprehend third party contractual arrangements.
• Requires basic computer skills including the ability to enter progress notes and other data in multiple medical record systems. Basic familiarity with Excel, Word, Outlook, and PowerPoint.
• Requires some knowledge of anatomy, physiology, and medical terminology.
• Requires some knowledge of managed care and government healthcare benefits.
