The primary job functions in Case Management include: Clinical Interventions/Discharge and Care Planning Management; Fiscal Management; and Payer/Referral Management. Case Managers duties include:

Current Licensure in a clinical discipline per state guidelines (RN, SW preferred)Preferred qualifications:
