The Case Manager provides age related case management to all consumers in the assigned program. Maintains and helps establish contact with referring agencies, other providers, and family-team members. Coordinates educational services for all residents of the program, and acts as educational liaison to the schools, handling educational and vocational needs. Is responsible for overseeing intake documentation is completed, and that all required documentation is collected. Directly coordinates the medical care for the consumers in the program, including verifying insurance information, obtaining late authorizations, identifying medical providers, and scheduling all necessary medical appointments; ensuring regulatory compliance on these items. Records all case management activity in the consumer’s medical record, and communicates it appropriately to all staff in the home. Participates in treatment team meetings with case management updates. Work with the psychiatrist in the residence to ensure all documentation by medical personnel is completed as required (including not limited to Consents for Medications and Pre-Treatment Clinical Assessments). Oversees the Medication Management System in the home, ensuring that all medications are sent to/received from pharmacy, and that all medications are available as prescribed to the residents. Reviews Medication Management System to ensure compliance with medication administration/adherence. Reviews this system with the nurse or other medical personnel reviewing the system. Demonstrates knowledge in job responsibilities, including the Electronic Health Record and Medication Management System, so as to act in a training capacity as needed (for this home or others). Works with the leadership team in the house to identify supportive therapy services for individual residence and conducts psycho-educational groups for the residents, ensuring that contract requirements are met weekly. Provides supervision services, transportation, and other services as required, to residents in the home. Accepts the responsibilities of the Safety Officer for the residence. Accepts On-Call duties as required, is part of the On-Call rotation. This is not all encompassing and other duties may apply as assigned.
