The Case Manager will work within the JYF non-secure detention continuum to effectively manage the intake, adjustment, advocacy, planning, discharge and re-entey of juvenile delinquents, juvenile offenders and adolescent offenders remanded to custody of ACS within an assigned non-secure detention facility. Specific duties include, but are not limited to:

Responsibilities Conducting intake while providing comprehensive case management services to assignedProgress notes must be completed within the first seven days for all new intakes OCFS required mandates.Maintain accurate client documentation and other pertinent paperwork/reports. Well organized process with shared space for full access to youth binders/ folders but limited access to PREA specifications and confidentiality.Provide concrete services, resources, counseling while collaborating with the clinical team, to develop and support a holistic approach.Submission of Mental Health referral where court orders specify the need for immediate treatment.Perform daily case management services (e.g., including intake assessment packet 56 pages to be completed for the youth and their guardian for initial screening. Must maintain a family visitors schedule submitted to ACS for each visit received. Monthly tracking for intake and discharge for accuracy on care days for each month. Facilitating youth access to authorized telephone calls) access to counsel does not require limit.Must attend court order proceedings as requiredProvide city court clerks with written adjustment reports approved by Management prior to submission within 48 hours of court appearances. Attend court sessionEnsure timely communication and ongoing collaboration with youth, their families, the courts, the youth’s attorney and other systems and agencies involved in the life of the youth.Participate in supervision and Mandatory training.Candidates are required to travel to other provider agencies for ACS mandatory Case Management meetings as well as school visits for provider meetings. Case Managers are required to work some weekends for special events, court ordered visits for families with small children and or working parentsAbility to work various hours and days of the weekPerforms other duties as assigned Qualifications Bachelor’s degree in criminal justice, Behavioral Health, Human Services, Social Work, or another related field preferred;Must have NYS Driver’s LicenseExperience working with youth and families in residential settingsExperience working within Child Welfare and/or juvenile justice fieldsGood written and verbal communication skills are essential.Computer literateUnderstanding the possible functions of behaviorsKnowledge of crisis intervention/de-escalation techniques

Starting salary $47,999/annually
