POSITION SUMMARY:

Person in this position will work in collaboration with community resources to provide services to participants/clients of WestCare Salem Veterans Home. Essential Job Functions include those listed below.

ESSENTIAL JOB FUNCTIONS:

Assess resource needs of participants. Educate and refer participants to community agencies and businesses that appropriately address the veterans needs and work on individual growth plan. Assist veterans with establishing or re-establishing support systems within their community. Act as advocate on behalf of the veterans to improve access to needed services with focus on VA. Participate in staffing decisions regarding the veterans current strengths and resources and their need for additional resources. Complete electronic documentation of services in a timely manner. Assists other WestCare staff as needed in a collaborative manner to promote cooperation and portray a professional image to the community. Must Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and Perform all other duties as assigned.
