Case Manager

At Independence Hall, we provide client-centered care and recovery-oriented treatment within a therapeutic environment. We are looking for innovative staff for our licensed Substance Abuse Treatment programs, which are open to both veterans and the community.\n

The primary goal of this position is to assist patients in obtaining necessary services by providing information, coordinating referrals, and following through with specific treatment plan goals. \n

WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY:\n\nProvides supportive case management necessary for individuals to access medical, social, vocational, educational, substance abuse and\/or mental health treatment supports and community-based recovery services. Assists clients with researching and accessing needed services.\nConducts referrals and follow-up monitoring, and participates in discharge planning process. Communicates with other service providers and family members, with appropriate releases, to coordinate the client's next level of treatment services.\nSchedules appointments and supports attendance at scheduled treatment and human services appointments. Coordinates with other providers to monitor clients' health status and participation in treatment.\nMonitors treatment progress and implementation of the client's treatment plan.\nConducts outreach activities to market Veterans Inc. programs and engage new referral sources.\nMaintains audit ready client files and documentation and facilitates client groups, as needed.\nAttends and participates in all meetings as required. Performs special projects and other duties as assigned. \n\n

WHAT YOU MUST HAVE:\n\nMinimum of Bachelor's degree in Human Services, Social Work, Psychology, or related field from an accredited institution. Master's degree in related field highly preferred.\nMinimum of two (2) years of case management, substance abuse treatment counseling experience required.\nDemonstrated knowledge of military culture, experience in working with veteran population and an understanding of veterans-related needs and issues preferred.\nTwo (2) years of experience facilitating groups or workshops preferred\nMust have strong computer skills with intermediate knowledge of Microsoft Office Suite.\nMust be able to successfully pass a CORI\/Background records check.\nCompliance with Veterans Inc. COVID-19 Vaccination Policy. Religious and Medical Exemptions are considered.\n\n
