JOB SUMMARY

The Housing Coordinator manages the Program’s housing supportive services, including emergency, transitional “bridge” and agreements.  Manages lease agreements including residential site leasing and master lease agreements.  Assists clients with documentation and housing placement. Manages a housing budget.

 

RESPONSIBILITIES

Locates housing opportunities in defined areas of the County that comprise the housing continuum including short-term, transitional and permanent options.Coordinates and oversees various kinds of oversight agreements, including master-leasing, scattered site leasing, sponsor-based leasing and assists in supervision of Assertive Community Treatment supported housing programs.Assists clients in obtaining necessary documents for housing placement, including completion of the Vi-SPDAT and CES entry.Will provide services in the field and provide on-call and after-hours services as required by contract or as assigned.Operates as a fully integrated member of the Assertive Community Treatment team. Interacts with the ACT team during Daily Organizational Meetings.  Works closely with program direct service staff to coordinate appropriate housing options for clients relative to functioning level and individual needs and preferences.Attends all required provider’s meetings in the community and represents MHS Assertive Community Treatment program in a positive and professional way.Acts as liaison with professional members of the housing industry as well as homeless and housing providers.Interacts with vendors to ensure all housing payments are made in a timely manner. Develops monthly reports for funding streams, and updates team on a regular basis regarding housing alternatives and changes in availability.Develops and runs housing invoice vouchers as outlined in program procedure for timely payment of clients’ housing, and tracks rent payments in collaboration with the Administrative Assistant.Comply with all MHS and program rules, policies and procedures including HIPAA standards.

QUALIFICATIONS

Bachelor’s degree and two (2) to three (3) years’ experience providing housing-related social services to clients, housing need assessment, search, and skills development in choosing obtaining, and maintaining housing permanence rehabilitation.   Must have knowledge of community resources and low-cost and subsidized housing.State of California licensed or license eligible (ACSW, PCCI, AMFT), preferred.Bilingual in English, Spanish and/or Vietnamese preferred. Experience working with the justice-involved population in a mental health / SUD setting or in a detention facility preferred.Proficient user of Microsoft Office Suite and EHR software.Ability to problem solve and react appropriately to emergency situations.Ability to work independently and effectively collaborate with a team.
