Location: Brooklyn, NY
Schedule: Monday - Friday 4 days 9 am - 5pm, 1 late day 11 am - 7pm, 1 saturday per month.
Case Manager ensures the best quality of care and services for program consumers in a Certified Community Behavioral Health Clinic (CCBHC). "CCBHCs are designed to provide a comprehensive range of mental health and substance use disorder services, particularly to vulnerable populations with the most complex needs".
SCOPE OF ROLE:
The ideal candidate will provide Case Manager services to clients within the scope of the clinic and be creative, innovative, open to change.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Provide case management to assigned caseloadDevelop and monitor individual service plans in conjunction with agency consumersAssist agency consumers with accessing needed community services (medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network, referral and follow upAssist agency consumers with problem solving and crisis interventionAccompany agency consumers on medical, psychiatric, and public benefit appointments, as necessaryAct as advocate for consumer as appropriate; encourage and support self-advocacyIdentification of any problems in daily living skills; hands on assistance and/or referral in addressing problems in daily living skillsMaintenance of a strong working relationship with all therapeutic support services to ensure the best quality of careDocumentation of all services in compliance with agency standardsWork as a member of the social service team in the development of off-site recreation, socialization, educational and skill building activities with a view towards community inclusion and integrationLiaison to community-based services and groups including civic, social service government, and business organizationsParticipate in ongoing individual and group supervision, team meetings, case conferences, division meetings and trainingsAdhere to on call cell phone system to facilitate crisis intervention, emergencies and other situations that require immediate actionThis role is comprised of 75% field work, travel is required, driving is not requiredOther duties as assigned.

QualificationsREQUIRED EDUCATION AND EXPERIENCE
Bachelor's Degree in Human Services or related field.2 years’ experience with individuals with mental illness, substance abuse issues and/or a history of homelessness.Working knowledge of various computer programs including e-mail, Microsoft Word, Excel, PowerPoint and Windows Operating System.PREFERRED QUALIFICATIONS & SKILLS
Master’s Degree in Human Services or related field.Bilingual English/Spanish is a plus.
