Case Manager
**Case Manager** **Hours:** 9a-3pm with flexibility Part-Time - 30 hours or more **Compensation: $20-22 per hour** **Position Summary:** The Case Manager works to make a meaningful difference in the lives of the children being served by the National Youth Advocate Program by working closely with the youth and family to provide advocacy and support towards targeted goals. **Working At NYAP:** + **22 Days Off** Each Year! **Plus 11** Paid Holidays Per Year! + **Summer Hours Off (reduced work schedule)** + **Student Loan Repayment assistance** , up to $1,200 per year! + Medical, Dental, and Vision insurance + Competitive salaries and benefits including a 401(k), Tuition Assistance, and **Work Anniversary Trips** ! + **Peace leave** + Mileage reimbursement for some positions. **Responsibilities:** + Provide advocacy services and support to RVPS participants (at risk youth, justice involved youth and survivors of community violence). + Assess immediate crisis needs and long term needs of participants and address appropriately by providing services and service referrals ensuring stability. + Manage calls and walk-ins and complete necessary documentation. Support the daily operations of the RVPS office. + Complete and maintain client records that document services provided. + Educate clients about dynamics of violence and assault and options/resources available. + Work with clients to develop and maintain relevant and meaningful case plans. Provide concrete case management and access to resources. Meet regularly with clients to support case plan accomplishment. + Refer clients to other agencies and resources and advocate directly with other agencies as needed to help client access necessary services. Maintain service partnerships with referral agencies. + Conduct assessments and intakes. + Facilitate crisis intervention and crisis management with Auburn Gresham community residents. + Provide appropriate follow-up and documentation of emergency incidents. + Participate in staff meetings, case consultations and other meetings as needed. + Facilitate weekly support groups for participants. + Perform other duties as assigned by Program Director. **Minimum Qualifications:** + **Bachelor’s Degree in Social Work or Human Services required.** + **Two years of experience providing case management services and care coordination.** + Experience providing victim services, outreach services, community programs, criminal justice or related experience. + Specialized training in assisting victims of violence crimes is desirable. **Knowledge of:** + Criminal and juvenile justice systems. + Victims' rights within the criminal justice system. + Needs and challenges experienced by people from diverse socio-economic and cultural backgrounds. Community resources and community organizations. + Various resources available to Peoria residents. + Crisis intervention methods and techniques. + Interviewing processes. + Restraining order processes and procedures. **Ability to:** + Communicate effectively, both orally and in writing. Interpret and explain policies, procedures, and relevant laws. + Conduct needs assessments and provide appropriate community referrals. + Communicate effectively with people from diverse cultures and backgrounds. + Maintain a high degree of sensitivity, tact, and diplomacy during stressful situations. + Prepare accurate reports and case summaries. + Maintain records and documents. + Make public presentations. + Maintain confidentiality with sensitive information. + Work emergency call-outs, nights, weekends, and holidays. + Establish and maintain effective working relationships with those contacted in the course of work. **Driving and Vehicle Requirements:** + Valid driver’s license + Reliable personal transportation + Good driving record + **Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability** If this describes YOU, please apply today! www.nyap.org/employment NYAP also requires all of our employees, regardless of your title/position, to hold and maintain automobile liability insurance coverage of $100,000/$300,000.
