PURPOSE
The Case Manager is responsible for case management and works closely within the Care & Assistance Team to address and coordinate the needs of students who have health, psychological, economic, family relationships, and/or social adjustment-related problems through a variety of interventions, referrals and follow up services. Provides consultation to faculty, staff, academic departments, and parents, when appropriate, to minimize academic disruptions and help resolve problems affecting students in distress.
DESCRIPTION
Functional Area 1: Case Management
Percent Effort: 50%
+ Address basic needs issues and wrap-around services for all students including parenting students, which includes addressing food and housing insecurities and financial, childcare, and academic needs.
+ Contact referred students, schedule meetings, assess current needs, and connect to resources.
+ Maintain confidential student records.
+ Complete documentation and maintain record keeping of Care & Assistance cases in Maxient.
Functional Area 2: Referrals and Follow Ups
Percent Effort: 25%
+ Manage and make appropriate referrals to on-campus resources and community-based resources.
+ Coordinate and follow up with students (email, virtual meetings, in-person meetings) regarding referrals and progress made on action plans.
+ Document progress and close cases within prescribed timeframe.
+ Provide support.
Functional Area 3: Support and Outreach
Percent Effort: 25%
+ Provide outreach and education to campus community
+ Provide support and outreach to staff and faculty when distressed students are identified, and support is needed.
+ Facilitate training and/or presentations to student, staff, or faculty.
+ Participate in outreach and tabling events for Care & Assistance, Dean of Students Office, and Student Engagement & Success Division.
Note: This position is grant funded. Continued employment is contingent upon continued funding.
Position’s annual work period is 9 months (September 1 – May 31).
Salary: $3,872.45 Monthly ($34,852 Annually Approximately)
QUALIFICATIONS
+ Bachelor’s degree in counseling, Psychology, Social Work, Student Affairs/ Higher Education Administration, or closely related field.
+ Five (5) years of related experience.
+ Experience in crisis management and response.
+ Experience in facilitating training and/or presentations to student, staff, or faculty.
+ Intermediate computer skills in Microsoft Word, Excel, PowerPoint, and Outlook. Intermediate knowledge of computer fundamentals, ability to use proprietary software and web-based computer applications.
+ Excellent interpersonal and presentation skills. Excellent verbal and written communication skills to be able to interact effectively with students, faculty/staff, community providers and parents.
+ Ability in managing difficult and complex situations involving students.
+ Excellent attention to details.
+ Knowledgeable about and skilled in working with a variety of mental and physical health providers and community agencies, crisis management/prevention, educational outreach to students, staff, faculty, and parents.
+ Ability to work cooperatively with others within a multidisciplinary team environment; evidence of effective collaborative relationships with students, faculty and/or staff.
+ Ability to develop and maintain case management files, referral resources and community services database.
+ Ability to maintain confidentiality based on relevant laws, policies and/or procedures.
PREFERRED QUALIFICATIONS
+ Master’s degree in counseling, Psychology, Social Work, Student Affairs/ Higher Education Administration or closely related field.
+ Knowledge of utilizing electronic case management system.
+ Experience working in a college or university setting in the area of case management.
+ Experience using a database management system such as Maxient, Symplicity, or a case management database system.
