Case Manager
Provides medical-surgical pediatric and obstetrical case management services to Brockton Hospital patients. Responsible for assuring continuity of care for patients under specific physician groups. Assesses plans, coordinates, and implements plans for quality, cost effective care for patients in the pre-admission, emergency, acute, and ambulatory settings. Is pro-active in the identification of high risk patients for timely case management interventions. There is collaboration with the attending and primary care physicians (PCP's) as well as other hospital clinical and administrative staff to develop and implement effective discharge plans. Monitors the appropriate use of hospital resources and facilitates the delivery of services. Uses accepted criteria and guidelines to conduct review of inpatient admissions. The Case Manager at Brockton Hospital serves as the primary liaison between PCP/attending physician, patient/family, payer based case managers, and appropriate community based providers. High school diploma or equivalent required. Bachelor's Degree required; may be waived with experience commensurate with job expectations. Master's Degree preferred. Must have current license from Commonwealth of Massachusetts to practice as a Registered Nurse. Minimum of five years clinical experience in an acute care facility required. Prior experience in case management, utilization review or discharge planning required.
