The Non-Clinical Case Manager coordinates referrals, service planning, and client documentation for designated caseload(s).
Essential Duties and Responsibilities:
Maintains full caseload, complete with ongoing individual service plans Assess ongoing changes in behavior, circumstances, or conditions that may affect client safety Engages /involves client and sponsor in the casework process Prepares, compiles, and submits accurate case files Serves as a liaison with stakeholders (i.e.. legal providers, immigration court) Actively participates in seminars, workshops, educational programs Completes home visits to meet with sponsor (if applicable) Maintains contact with clients through phone calls and home visits (if applicable) Consults with supervisors to determine decisions as they relate to the safety, well-being and permanency of patient Demonstrates client progress toward risk reduction, achievement of treatment goals and positive case outcomes Maintains accurate records, files, forms, statistics and additional relevant information in accord with agency policy, licensing, and/or funding requirements Conducts in-home assessments (if applicable) Performs other duties as assigned/necessaryMinimum Requirements:
Professional License in Behavioral Sciences, Human Services, or Social Services preferred Bachelor’s Degree in Behavioral Sciences, Human Services, or Social Services fields preferred Experience in Case Management, Behavioral Sciences, Human Services or Social Services preferred Complies with all relevant professional standards of practice Participation and completion of Amergis’ Competency program when applicable Current CPR if applicable TB questionnaire, PPD or chest x-ray if applicable Current Health certificate (per contract or state regulation) Must meet all federal, state and local requirements Successful completion of new hire training as applicable to job site Understand patient confidentiality and HIPAA requirements Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required Computer proficiency required Must be at least 18 years of age Benefits At Amergis, we firmly believe that our employees are the heartbeat of our organization and we are happy to offer the following benefits: Competitive pay & weekly paychecks Health, dental, vision, and life insurance 401(k) savings plan Awards and recognition programs *Benefit eligibility is dependent on employment status.
