What you'll do
The Case Manager is the primary contact between residents and program goals. This position is responsible for executing assessments, documenting results and progress in the client management system, actively collaborating with community resources, communicating routinely with all participants on their case load, providing direct services and coordinating referrals to external agencies for services. The Case Manager assists participants in moderating the effects of incarceration, substance abuse, financial distress, family needs, housing, and mental health.

Primary/Essential Duties and Responsibilities are but not limted to:

Assess individual risk, needs and barriers using validated tools and techniques, assure client and facility safetyMaintain professional boundaries with clientsDeescalate clients in need of assistanceDevelop and maintain working knowledge of community resourcesAssists first responders as needed (i.e. police and medical personnel)Complete and document a resident needs assessment upon intake Collaborate with Community Recourses to meet resident needsFacilitate relevant group sessions to meet resident needs Work with Program staff to assess resident behavior and needsCoordinate resident entry into needed community programs, such as detox, residential treatment, housing, etc.   Routinely contact each resident based on the appropriate level of serviceConduct individual goal-setting and case-planning sessions with each residentUtilize motivational Interviewing (MI) practices Utilize results of needs assessment and other information to assist with completion of short-term and long-term goalsReview and monitor case plans with client, documenting progress Document all assessments and progress in efforts to outcomes database including: progress notes, needs assessment, barrier tracking at scheduled intervals, direct services provides, external referrals coordinated, and other relevant information.

 

What you'll bring

Bachelor's degree in social work, psychology, criminal justice or related fieldOne year of experience working with marginalized populations  Valid Washington State driver's license.Employees who operate their own or PHS vehicles on PHS business, must have a safe driving record as defined by PHS.Employees who operate their own vehicle(s) on PHS business must carry auto liability insurance that complies with the requirements of PHS.

Must satisfactorily pass tuberculosis (TB) test

 

Preferably you'll bring

1 yr. experience in case management experience preferably in a residential program
