As a Case Manager you will coordinates and oversee patient care, ensuring timely and cost-effective treatment. You will assess patient needs, develop personalized care plans, and facilitate communication between healthcare providers, patients, and families.

Key Responsibilities:
Assess patient needs: Evaluate patient's medical, social, and emotional needs.Develop care plans: Create personalized plans to achieve patient goals and outcomes.Coordinate care: Arrange and facilitate healthcare services, appointments, and treatments.Communicate with stakeholders: Collaborate with healthcare providers, patients, families, and insurance companies.Monitor progress: Track patient progress, identify barriers, and adjust care plans as needed.Advocate for patients: Ensure patient rights, interests, and preferences are respected and addressed.Manage resources: Optimize resource utilization, reduce costs, and improve patient outcomes.Education and/or Experience Qualifications:
A Bachelor’s Degree in a Human Services field from an accredited college or university.Minimum of two (2) years of relevant work experience.Additional Qualifications:
• A valid driver's license and a reliable vehicle.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Flexibility and adaptability in a fast-paced environment.
