Job Description
They will be joining a team of 18 case managers. This person will work in the field, meaning 75% of their role will be spent meeting patients for appointments at doctors' offices, physical therapy, hospitals in downtown Philly. Case Managers have 1-3 appointments a day with a case load around 18-23 files, depends on experience. The other 25% of their week will be remote coordinating with patients and providers via email/calls and documenting. While in the field Case Managers will be commuting between appointments, they will need their own care and experience navigating downtown. They will be reimbursed for their milage and parking. Case Manager are eligible for a productively bonus based off their 3 month average of billed invoices. Case Manager need to be helpful and passionate about working with patients.

2+ years well rounded RN background
2+ years general bedside experience
Computer skills, documentation, Microsoft
experience writing professional reports
person transportation
comfortable navigating downtown
comfortable walking to appointments in downtown Case Management Experience
Rehab or Orthopedic background null
