Earn $22.75 per hour with benefits working a 40 hour week.

 

About the position:

This is a case management position working with adults in a transitional housing environment. Our service populations are in the early stages of recovery from chemical and alcohol dependency, co-occurring disorders, serious mental illness, and homelessness.

 

Duties:

Case manages the needs of program residents. Link residents to various community based services and resources as needed. Lead a case management team of supportive service coordinators and facility attendant 2’s and make recommendations to the program director. Ensure all progress reports are completed properly and on-time. Incident reporting to include writing incident reports, reporting to immediate supervisor and following procedure as stated in Merakey policy.  Encourage and verify that residents with income are saving money. Data input and outcome analysis. Provide training as needed and directed by program director. Work with the program director to develop and enforce policy and procedure. Life Skills Group facilitation (i.e. money management, housing acquisition, nutrition). This position may also be asked to assist with interviews, intakes and various other tasks in an effort to support program operations and the program director.
