The Case Manager is responsible for the direction, provision, and quality of social services provided for members and families. Duties include all Social Service Assessments and Summaries and provide case management services to address the psychosocial and socio-economic needs of our members and their families. The Case Manager will ensure that all work complies with company policies and procedures, safety, and regulatory laws and standards.

Responsibilities Oversees and participates in activities of the Social Work Department.Provides case management to coordinate with internal and external ancillary services for members.Ensures compliance with all company policies and procedures and applicable regulatory authorities (i.e., MA Department of Health and Senior Services, state and federal environmental agencies, OSHA, USDA) and prepares paperwork as required.Participates in Safety and Quality Assurance Committees and establishes and schedules member’s Council meetings, Caregiver Support Groups. Participates in Care Plan for Interdisciplinary Team and records clinical notes as required.Maintains formal written communication with members and families and facilitates counseling through support groups and one-on-one sessions.Maintains accurate files and records of the members and social services information. Provides billing, transportation, and member’s referral information as needed.Participates in community outreach programs, including communication with other health and social service providers and inquiries from prospective clients. Participates in facility marketing activities.Provides active assistance with the member’s as needed (i.e.: hygiene, meals, ambulation, arrivals and departures, and social activities).Other duties or responsibilities that management may deem necessary from time to time.

Qualifications A degree in human services or a related field from an accredited college or university and at least one year of relevant and recent experience working with adults in a professional capacity, or MSW degree, is required.Two years experience in a health care facility, preferably working in a senior or long-term care environment. May be required to be bilingual (English/Spanish).Knowledge of company policies and proceduresKnowledge of MA Dept of Health and Senior Services Manuals of Standards for Adult Day Health Care LicensureKnowledge of applicable state and federal environmental and OSHA regulationsAbility to demonstrate excellent English communication skills, both orally and in writingAbility to demonstrate excellent interpersonal skillsAbility to demonstrate excellent organizational skillsAbility to make independent decisionsAbility to handle multiple prioritiesAbility to motivate and train employeesAbility to lead and direct the activities of othersAbility to work in a cross-functional capacityAbility to use a personal computer
