Job Description Good Shepherd Fairview Home is in search of a caring individual to promote person-centered care for optimal quality of life for our residents as our Case Manager- Assisted Living Position Details Monday - Friday, Day shift; Occasional after - hours/ weekend Position Benefits Benefits with full-time employment include; Medical, Dental & Vision insurance. Position Summary + The purpose of this position is to provide case management services as are necessary to support the resident in maintaining independence of function and personal choice. + Identify and evaluate the resident’s needs, interests, and strengths and the capability of the facility to meet those needs, prior to admission and then at least once every 12 months, + Oversee and coordinate a written Individualized Service Plan for each resident; in compliance with federal, state and local regulations and to promote person-centered care for optimal quality of care Position Qualifications/ Requirements Bachelors or Masters with major work in human service delivery and one year of full-time experience in the provision of services to a dependent adult population Or Associates degree with major work in human service delivery and 3 years full time experience in the provision of services to a dependent adult population APPLY NOW!
