Case Manager
Downtown Streets Team (DST) is a nonprofit organization committed to fighting homelessness through the power of work and dignity of community. DST increases social capital and economic mobility by helping our unhoused neighbors chart a way out of homelessness through community building, volunteering opportunities, employment and human-centered case management. Since 2005, DST has expanded across 16 cities in Northern and Central California, serving over 1,500 people annually. SUMMARY: We’re looking for a full-time Case Manager to work directly with program Alumni who have obtained stable housing, in addition to current Team Members (our unhoused clients) to navigate social services and reduce barriers to self-sufficiency. This may include helping Team Members access shelter, employment, housing, medical, legal, financial services, and other goals. As the primary Case Manager supporting Alumni, you will be responsible for hosting monthly meetings, regular workshops and events to provide on-going support to graduates. You’ll need strong organizational, problem-solving, analytical, programmatic, and people skills while managing priorities and workflow. WHAT YOU'LL DO: Help Our Team Members Achieve Self-Sufficiency Provide compassionate support and assistance to each Team Member to achieve their goals on housing, jobs, transportation, health, and legal issues Work with Team Members to develop job search collateral, run employment readiness workshops, and employment-focused volunteer pairing events Develop housing and employment leads with local landlords, property managers and employers Utilize any available subsidies and affordable housing units to place Team Members into housing Maintain regular appointments and office hours with Team Members and staff Coordinate with Lead Case Manager to plan and schedule home visits for housed Alumni to check-in on well being and case management needs Develop and implement individualized aftercare plans tailored to each alumni’s needs, focusing on stability, growth, and self-sufficiency Present new services and opportunities to the entire Team at designated Case Management sections in weekly Team Success Meetings Coordinate with office administrator and other case managers to plan celebrations for Team Members completing program during success meeting Facilitate engaging monthly meetings for alumni, creating a supportive environment that fosters a sense of community, share successes, and encourages peer support Track data specific to Alumni program and work with program manager to prepare monthly reports to funders Support Team Members in court, at housing related appointments, social service agencies, etc. This may include providing them with transportation Establish productive relationships with each and every Team Member with no bias or prejudice towards race, religion, age, sex, disability, or sexual preference Work Well With Others in DST & the Community Work with the Program Manager to promote successful participation in the work experience program Actively document client meetings and update all relevant program data in applicable databases Uphold organizational values, work ethically and with integrity, and follow company policies as laid out in the employee handbook Represent DST in the community, and work with all partner organizations to achieve client goals Distribute outreach supplies to individuals in encampments, RVs, and on the streets, and promote participation in the work experience program Actively learn about current resources and services outside of DST that may aid Team Members in their goals Perform other duties as assigned by manager and assist with special projects as required WHAT YOU'LL NEED: Ability to perform job duties with minimal supervision Excellent communication skills, with fluency in written and spoken English Basic mathematical calculations Problem solving and observation skills Self-starter with strong organizational skills with the proven ability to multi-task, prioritize duties, and manage time Works well with others and behaves professionally and ethically Solution-oriented Ability to work in a changing environment High school diploma/GED Reliable vehicle with current insurance, and the ability to travel regularly throughout the local area Physical requirements: sitting, standing, or walking for long periods of time and pushing, pulling and/or lifting up to 25 lbs.
