Case Manager

OVERVIEW: 

The Case Manager is responsible for screening and assessing prospective program participants, including conducting intakes and evaluations related to substance abuse and mental health needs. This position provides counseling and program services to help clients achieve stability, increase their income levels, and gain greater self-determination.

DUTIES AND RESPONSIBILITIES:

Conduct intakes and assessments Provide case management support and other services Lead mental health and substance abuse disorder groups and individual cases Maintain accurate records of all group meetings, resident participation, and attendance Work with referring agencies and community partners Lead psychosocial and psychoeducational classes Facilitate process group and life skills class Provide referrals for clients needing medical, mental health, and substance use issues Attend and participate in staff meetings Evaluate, record, and report on client's progress Drive to shops for food and supplies Other related duties as required

EDUCATION AND EXPERIENCE:

High school diploma or equivalent (required) Experience in case management (1 year required) CA driver’s license (required) 21 years of age (required) Bachelor's degree in Social Work or related (preferred) Certification in drug and alcohol in the State of California (preferred) Experience with Microsoft programs Experience working with veterans

Modern Office Equipment and Relevant Software

PHYSICAL REQUIREMENTS:

Ability to lift up to 25 lbs. (usually file boxes)
