Case Manager

The Case Manager will:

Ensure Medical Needs are met and maintained for youth Ensure Foster Parents are trained on Medication Policies and Procedures including TFC Medication Manual Provide additional re-trainings as needed regarding the medical/ medication needs to Foster Parents Work closely with the prescriber to communicate any psychiatric or behavioral concerns regarding the consumers in care. Schedule monthly medication monitoring for incoming consumers as well as ongoing for consumers who are prescribed psychotropic medications Maintain communication with outside agencies and/or other parties involved Attend Treatment Team meetings to provide medical information/support as well as add therapeutic input Provide therapeutic support to clinician Serve as on-call staff to provide therapeutic support to foster parents with crisis after hours Assist with consumers at SCIP/Crisis Unit on an emergency basis Assist with consumers in home or in office when behavioral issues arise Maintain electronic medical records Complete required annual trainings Resolve pharmacy issues Assist with intake process as needed Transportation as needed (medical appointments and other) Provide education to consumers regarding proper hygiene Complete incident reports Medication maintenance at intake as well as discharge Medication Disposal as needed

Position is full time and includes the following:
