Our client, a leader in healthcare services, is seeking a Case Manager to join their team. As a Case Manager, you will be part of a multidisciplinary team supporting individual and small group members. The ideal candidate will have strong communication skills, a collaborative mindset, and a commitment to patient-centered care, which will align successfully in the organization.
**Job Title:** Case Manager
**Location:** Durham, NC (Remote)
**Pay Range:** $41/hr on W2
**What's the Job?**
+ Serve as a team member on a multidisciplinary team, coordinating care, resources, and services for members to achieve optimal clinical and resource outcomes.
+ Utilize applicable clinical skillset and perform comprehensive assessments to determine how to best collaborate with members, family, internal partners, and external services/providers on plans for treatment.
+ Develop a member-centric plan tailored to members’ needs, health status, educational status, and level of support needs.
+ Perform ongoing monitoring and management of members, including scheduled follow-ups and assessment of progress towards goals.
+ Educate members and encourage proactive intervention to limit expenses and promote positive outcomes.
**What's Needed?**
+ RN, RD, or LCSW with 3 years of clinical and/or case management experience required.
+ Must have and maintain a valid and applicable clinical license (NC or compact multi-state licensure).
+ Experience in MS Office and electronic documentation systems is required.
+ Hospital background or case management experience is helpful.
+ Ability to effectively document all aspects of the care plan and member interactions.
