Case Manager

Job Description

We are seeking a dedicated Case Manager to join our team. This role involves communication and relationship-building with clients and all collateral partners connected with each client, including schools, funding partners, therapists, agencies, and foster homes. You will be responsible for setting limits for youth, developing interventions, and setting goals toward individual successes in conjunction with clients' guardians, therapists, and other providers. Participation in on-call services as needed is also required.

The pay range for this position is $52000.00 - $52000.00

This is a fully onsite position in Malden,MA.
