CASE MANAGER

Pay Rate: $22.00

Shift: Monday-Friday 8:30am-4:00pm 

 

Assists those 60 and older with offering information and assistance and access to necessary services and supports appropriate to live independently. Will screen, assess, develop and implement a care plan with the individual and provide referral/linkage to appropriate programs and services. Follow up and monitoring of the delivery of services will take place as required with re-assessment when appropriate. Plan and implement outreach activities in the community.

ESSENTIAL FUNCTIONS/RESPONSIBLITIES
• Provides information and referral on services to the elderly, their families and other service providers on the telephone, in person and through community forums.
• Conducts thorough intakes of clients to determine the need for case management services.
• Is responsible for in-home assessments of clients and development of care plans to meet needs. Implements care plans and revises as necessary.
• Advocates on behalf of clients with formal and informal support systems. Provides linkages and referrals to agencies and programs providing assistance to seniors. Stays current with programs pertaining to the elderly.
• Attends meetings, trainings and in-services as required by the Erie County Department of Senior Services.
• Demonstrates the ability to successfully utilize computer software for assessments, reporting (including worker log) and correspondence. including email, scanning, faxing and other forms of communication, such as virtual conferencing.
• Is responsible for maintaining client case records in an accurate and timely manner. These records will reflect understanding of and adherence to established EISEP standards. Case Managers will maintain a caseload of approximately 100 clients.
• Participates in regular case conferencing sessions with supervision and consults with supervisor as needed on difficult cases.
• Intervenes in crisis situations, making appropriate referrals to agencies dealing with medical, environmental, mental health and utility emergencies.
• Maintains a working knowledge of community resources to meet the needs of clients.
• Work is performed under the direct supervision of a Senior Case Manager, Social Case Supervisor or and administrative employee of higher rank. Supervision may be exercised over lower ranking employees. Does related work as required.
• Other duties deemed as appropriate by Project/Assistant Coordinator
• Supervises students, volunteers, aides or other individuals as assigned.
• Maintains confidentiality; utilizes potentially sensitive information in accordance with agency and departmental policies.

MINIMUM QUALIFICATIONS

• Bachelor’s degree required.
• One year experience preferred.
• NYS Drivers License that meets agency policy.

SUPERVISORY RESPONSIBLILITES:

Responsible for oversight of students, volunteers, aides or other individuals as assigned.
