Provides housing case management, placement, stabilization/retention and budget counseling services to individuals, youth and families referred for services. Meets with the clients in their homes.2. Conducts intake assessment and service planning for clients in a timely manner, following all contractual requirements.3. Coordinates services with case managers at the Department of Children and Family Services (DCFS) and other agencies. Refers clients to internal and external resources and ensures coordination of services. Develops and maintains relationships with landlords and affordable housing resources. Advocates on behalf of clients in order to ensure access to services for which the client is eligible.4. Maintains up-to-date and accurate client files and databases, as required by agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing or accrediting bodies.5. Assesses funding availability for clients in compliance with DCFS and agency policy and procedures. Documents all funding provided according to the program procedures.6. Assists clients in obtaining needed household items from local community resources. Distributes donations to clients; lifting and carrying items in delivery to clients.
