The Case Manager will work for our Seniors Division, and will provide case management services to seniors ages 60 years old and over. This is a great opportunity for a recent college graduate! Responsibilities + Provide an initial assessment of the senior adult's needs and develop a care plan for services and assist with the implementation of this plan. + Provide monitoring and reassessment of all open EISEP case management cases to assure that all services are being delivered as planned + Prepare reassessment forms for all EISEP clients receiving home care services + Assist with the EISEP Case Management Program office coverage biweekly and as needed. + Provide information and referral to clients as requested. + Provide advocacy for clients regarding entitlements and other basic needs Outreach within the community to provide information and assistance. Qualifications + Bachelor's Degree required + Minimum of 1 year of case management experience + Experience with geriatric population preferred + The ability to do home visitation + Excellent verbal and written communication skills + Proficient computer skills with MS Office + Valid and clean driver's license Additional Information Schedule: + Full-time, 35 hours per week, Monday-Friday + Salary: $26-28/Hour + Location: Garden City Compensation Information + FCA's framework, culture, and values reflect the competitive compensation range.
