Case Manager
Job Title: Clinical Patient Care CoordinatorJob Description We are looking for a dedicated Clinical Patient Care Coordinator to join our team. The successful candidate will be responsible for rooming patients in a clinical environment, taking vitals, and coaching individuals on healthy lifestyles. This role requires the ability to manage a diverse case load and work on your feet for long periods of time. Responsibilities + Room patients in a clinical environment. + Take patient vitals. + Coach individuals on healthy lifestyles. + Manage a diverse case load using EMR software. + Schedule patients for follow-up visits as needed to progress towards goals. Essential Skills + Experience in patient care. + Ability to take vitals. + Experience with appointment scheduling. + Proficiency in using electronic health records (EHR). + Ability to manage a diverse case load. + Comfortable working on feet in a fast-paced environment. + Experience in case management and managed care. Additional Skills & Qualifications + Experience in dietary education. + Previous experience working with tribal health is a plus. Work Environment Clinic environment working from 8 AM to 5 PM. Personal protective equipment (PPE) including gloves, surgical masks, and medical supplies such as MMR, Varicella, and TB are provided. Pay and Benefits The pay range for this position is $32.00 - $38.00/hr. Workplace Type This is a fully onsite position in Oklahoma City,OK.
