Hartford HealthCare network and is the systemrsquos second largest hospitalJob SummaryThe Behavioral Health Case Manager provides intensive non clinical case management services as an integral member of patient care teams in the Emergency Department Behavioral Health Unit The Case Manager works collaboratively with the patient and their treatment providers to overcome any barriers patient may encounter while accessing our services The Case Manager provides trauma informed case management to support patients and families in order to assist patients in achieving their treatment goals and objectives during treatment They help coordinate transitions of care within the department andor to outside agencies Provides critical information to members of the patientrsquos clinical care team to aid in monitoring and adjusting the treatment plan while working closely with the patient parent or caregiver Advocates and represents the voice and needs of the patient when securing services with local state and federal agencies assists the patient in obtaining any basic needs that they or their family may need including but not limited to outpatient behavioral health treatment re connection to current mental health providers information and access on food stamps WIC and other food services transportation out of emergency department and community assistance by referral to proper resourcesResponsibilitiesProvide non clinical case management services on behalf of the Clinical Care Teams Case management support may include some or all of the following responsibilities locating outpatient behavioral health services which may include substance detox or rehab beds collateral contacts related to insurance arranging transportation communication with health care providers securing school and agency schedules setting up medical and behavioral health appointments gathering collateral medicalpsychiatric records routine non clinical correspondence providing information on meeting basic needs foodclothingshelter and access to healthcare care connection calls with parents and patients collaboration and support of family members or guardians and coordination of care between patient family caretakers hospital and community providersAttend and participate in Clinical Care Team Meetings safety huddles mandatory departmenthospital meetings and assigned hospital community or hospital system sub committeesComplete all assigned documentation responsibilities for collateral contacts that meets established Joint Commission state licensing and corporate compliance criteria Complete all assigned tasks related to quality assurance data collection and data entry as required by the department grant sources state agencies and insurance bodiesRounding on patients to support and respond to questionsneeds at intervals determined by the needs of the department where assignedCompile and maintain electronic and hard copy resource manuals for patients parents
