What you'll do
As a member of the PHS Housing team, we are committed to the mission and vision of providing supportive services that promote proactive social and independent living skills while employing effective business practices that support the success of the housing division. The Case Manager is responsible for the delivery of supportive services to participants of City Gate Apartments, such as creating and monitoring Individual Service Plan (ISP) for each participant to include career laddering, wage progression, food/benefit security among others. The Case Manager is also responsible to serve as liaison with community resource agencies to coordinate needed services.
What you'll bring
Bachelor’s Degree. Relevant experience in the human services field may be substituted for educational requirements.One-year experience in human services, preferably working with homeless adults, the economically disadvantaged, mentally ill or chemically dependent.Current CPR/First AID Certification or within two months of hire.Valid Washington State driver's license.Employees who operate their own or PHS vehicles on PHS business, must have a safe driving record as defined by PHS.Employees who operate their own vehicle(s) on PHS business must carry auto liability insurance that complies with the requirements of PHS.
Preferably you'll bring
MA/S degree in social or behavioral science preferred.
