With the support of more than 30,000 team members, we promote a forward-thinking philosophy that’s built on a foundation of trust, dignity, respect, responsibility and clinical excellence. **Facility:** BayCare Health System, ASA Outpatient-BHA **Location:** Behavioral Hlth Doris Cook Outpt **Status:** Full Time, Exempt: No **Shift Hours:** Monday-Friday: 8AM - 6PM **Weekend Work:** None **On Call:** No **The Case Manager is responsible for:** + Case Managers provide direct behavioral case management services to adults or children with serious mental health conditions, substance use disorders, and/or those whose involvement in the child welfare system requires. + The Case Manager should demonstrate competency in the domains of Engagement and Assessment; Service Planning and Delivery; Coordination, Linkage and Monitoring; Documentation; and Professional Responsibility. + The Case Manager coordinates the service delivery for individuals, and provides community partners with up to date available referral resources to provide continuity of care. + Case Managers effectively maintain and manage their assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served. + The Case Manager possess a clear understanding of the service delivery system in accordance with regulations and district policy. + Case managers provide services at the time and location that is most appropriate to the person served. Travel is required. + Case Managers may also be required to conduct urine drug screening and transport clients, depending on the program. + Some programs may also require a 24 hour on-call rotation. Case Managers will perform other duties as assigned. **Required Education:** + Bachelor’s degree related to related fields, Social Work, Counseling, Psychology Criminal Justice, Health Education, Special Education OR Other Human Services Related Field. **Required Experience:** + For targeted case managers working with children have 1 year full-time experience working with children with behavioral health concerns. + For targeted case managers working with adults have 1 year full time experience working with adults with behavioral health concerns. + **Preferred Experience** : 1 year of full-time experience with the identified population **Required Certification/License:** + BLS (Basic Life Support) + Driver's License + For targeted case managers (CBHCM) Certified Behavioral Health Case Manager certification must be obtained through the Florida Certification Board (FCB) within one year of employment.
