Case Manager (BJA) Job Details Job Location Newport - 219 East Broadway - Newport, TN Position Type Full Time Education Level 4 Year Degree Salary Range $22.00 - $24.00 Hourly Travel Percentage Up to 25% Job Shift Day Description Position Summary: The Case Manager serves as a member of the Treatment team of WestCare Tennessee and will primarily be responsible for working collaboratively with the Bureau of Justice Assistance program’s Project Director, Nurse Counselor and Peer Support Specialist; in addition to, Cocke County Jail staff, criminal justice supervising authorities, treatment professionals, and County District Court Judges. The Case Manager will provide services to client or participant s and meet the needs of the client or participant to determine appropriate referrals. Essential Job Functions: + Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Tennessee. + Uphold strict confidentiality of client or participant information. + Align clinical and treatment approach with WestCare Tennessee values, ethics, and code of business. + Become trained in Moral Reconation Therapy (MRT), Relapse Prevention, Parenting, Job Readiness, Breaking the Chain of Trauma, Anger Management, Seeking Safety. + Deliver curriculum to client or participants in a group setting. + Participate in team meetings, contributing to discussions about treatment planning, client or participant progress, and clinical concerns. + Maintain current knowledge of state and federal regulations related to mental health and substance use care. + Perform grant required assessments to help provide a comprehensive overview of current needs of the client or participant to the Nurse Counselor and Clinical Director. + Make recommendations to the Clinical Compliance Director and TN Regional Administrator on staffing to provide safe and effective care to client or participants. + Educate and refer client or participants to community agencies and businesses that appropriately address the client or participant needs for food, shelter, clothing, employment, transportation, and other community resources. + Assist client or participants with identifying and accessing non-clinical recovery support services as part of treatment and transition plans. + Assist in implementing a public health approach that blends community-based prevention with clinical strategies through community efforts. + Maintain strong professional relationship boundaries with individuals served and organizations/ community agencies who refer individuals to WestCare Tennessee. + Review all curricula, training resources and program material for cultural and linguistically appropriateness. + Attend necessary monthly integrated staff team meetings. + Assist client or participants with establishing or re-establishing support systems within their community. + Advocate on behalf of the client or participant in order for them to receive access to needed services in addressing appropriate needs. + Complete documentation of services on provided forms promptly. + Attend necessary monthly integrated staff team meetings. + Maintain strong professional relationship boundaries with client or participant s served. + Submit weekly report to Project Director. + Provide input to the BJA Nurse Counselor regarding the client or participant treatment needs which may include conducting discharges, incident reports, and other related documentation. + Perform client or participant follow up interviews to document progress, to make additional referrals if needed and to maintain active communication throughout the duration of their enrollment in the program. + Perform additional tasks that support a productive office workflow as deemed necessary by the BJA program guidelines. Qualifications Essential Qualifications: Certifications/Licenses: + Valid state driver’s license is required. + CPR Certification, First Aid Certification Education: + Graduation with a four-year degree in Addiction Counseling, Psychology, Social Work, Community and Human Services, Rehabilitation Counseling, or related field is required. + 2-4 years of experience in related project field may be substituted for degree. Experience and Competencies: + Case management experience preferred. + Ability to work collaboratively with others in a professional manner. + Ability to work well in a team environment. + Ability to exercise sound judgment and discretion. + Ability to operate a computer and complete documents in Microsoft Word and Excel program formats. + Ability to respond as needed to a crisis in an efficient manner. + Ability to complete work tasks within scheduled work hours. + Possess excellent verbal and written communication skills. Working Conditions: + Work is performed in a combination of an office and outdoor setting. + Some travel is required.
