Job Title: Case Manager - Critical Need
Job Description

Review admission reports and coordinate with hospital social workers and discharge-planning staff to ensure early identification of high-risk patients and timely discharge planning. Identify, facilitate, and advocate appropriate organizational and community resources to meet the plan of care and ensure that they are implemented in a cost-effective, efficient, and timely manner. Analyze patient and program outcomes to identify improvements in program quality and cost-effectiveness of case management activities. The clinician must be self-motivated and able to learn in a fast-paced environment and work independently once orientation is completed.

This position requires working onsite at VHC. The shift is from 08:30 AM to 05:00 PM with every other weekend and holiday commitment. The role will involve working with two EMR systems: KP EMR and Vendor EMR. This opportunity includes great health benefits upon conversion and is a day shift role with a top employer in the country.

Pay and Benefits

The pay range for this position is $45.00 - $55.00
